A massage membership is a commitment to your health. No-long term contracts. Memberships auto-renew on a monthly basis. Choose from (1) 60 Minute massage per month, (2) 60 Minute massages per month, (1) 90 Minute massage per month or (2) 90 Minute massages per month.

If you're ready to start a membership, you can review the agreement below. Print and fill it out, bring it to your next appointment and I'll get you all set up!

The intake form provides me with relevant contact information, medical history, and goals for massage therapy. With this information, I'm better able to customize your sessions.
